Non-Licensed Legal Staff Registration

This form is to be used by non-licensed legal staff to register with the Illinois Department of Corrections for access to the
Department’s Multi-Factor Authentication Mail System to send correspondence to individuals in custody. You must provide the Sender ID of the licensed attorney
on whose behalf you will be mailing correspondence.

By registering for use of this system, you will be responsible for any correspondence sent to an individual in custody using a Control
Number you generate.
